Mandaue City is located on the central - eastern coastal region of Cebu. C. Affiliated Hospitals, Physicians and Diagnostic Clinics/Center – are accredited health providers that cater to the health needs/services of our Card Holders.
D. Sales Associates – persons who are duly licensed by the company upon completing the prescribed requirements to sell the company’s products.
E. Sales Associate’s Code – a unique personal number given to a licensed Sales Associate as his/her official identification.
F. Employee – a person employed by CareHealth Plus.
G. Branch Code – is an exclusive number that is given to each CareHealth Plus’ branch nationwide for official identification.
H. Data Subject – is an individual’s personal information which is processed by CareHealth Plus.
IV. Collection of Data
CareHealth Plus reserves the right to collect, store and use the following kinds of information:

3. Branch
4. Branch Code
CareHealth Plus reserves the right to collect information from its users when users browse its website.
