A. Branch – is a detached office location manned by a branch manager and usually by a registered nurse. It is the official extension of the company.
B. Card Holder/Member– a person who a CareHealth Plus’ health care plan.
C. Affiliated Hospitals, Physicians and Diagnostic Clinics/Center – are accredited health providers that cater to the health needs/services of our Card Holders.
D. Sales Associates – persons who are duly licensed by the company upon completing the prescribed requirements to sell the company’s products.
E. Sales Associate’s Code – a unique personal number given to a licensed Sales Associate as his/her official identification.
F. Employee – a person employed by CareHealth Plus.
G. Branch Code – is an exclusive number that is given to each CareHealth Plus’ branch nationwide for official identification.
H. Data Subject – is an individual’s personal information which is processed by CareHealth Plus.
IV. Collection of Data
CareHealth Plus reserves the right to collect, store and use the following kinds of information:
1. Sales Associate Surname
2. Sales Associate Code
3. Branch
4. Branch Code
CareHealth Plus reserves the right to collect information from its users when users browse its website.

- Access of training modules by Employees and Sales Associates to learn the different products offered by CareHealth Plus

CareHealth Plus follows strict implementation of industry standards in terms of its technical and organizational safety measures to secure the availability, confidentiality and integrity of any personal information collected from the website.
